Patients often ask me this when they’re trying to build a consistent vitamin routine. My honest answer: it depends on the vitamin.
Fat-soluble vitamins — A, D, E, and K — are absorbed better with food, ideally a meal containing some fat. Taking these on an empty stomach won’t harm you, but you’ll likely absorb less of them.
Water-soluble vitamins, like vitamin C and most B vitamins, are generally fine on an empty stomach. However, some people find they cause mild nausea or stomach upset without food in their system.
Iron supplements are a special case — they’re absorbed best on an empty stomach, but this is also when they’re most likely to cause nausea or stomach discomfort for many people.
My advice: If a supplement upsets your stomach, take it with food — better absorption of a lower dose beats poor tolerance of a “perfect” one. Consistency matters more than perfect timing.
